This is the current valid As Is Buyers Guide form. 
These buyers guides feature two permanent adhesive tape strips; one on the top and one on the bottom, which adhere to the inside of the car window. Simply remove the two strips of backing paper and stick to the inside of the vehicle's window to apply. The second part provides an additional copy for added convenience.
2-part, carbonless (NCR) form is comprised of white, white.
This is an English language form.
White forms are printed with black ink on the front and gray ink on the back.

Includes lines to list the systems covered and duration of the warranty, selection boxes for Non-Dealer Warranties and Manufacturers Warranties, and the standard buyers guide text on back that fulfills the FTC requirements with room for the client signature.
